Captivate 6 Video not displaying

I have a web course generated in Captivate 6 that had been working fine; but now does not display video after the first slide that has video. The audio for the video plays fine; but the video doesn't display. Has anyone else had this experience? The program was published using the HTML5 format.

How was the video added?

Inserted as a file to Cp?

Using the WebObject and an Embed url?

Using the YouTube widget?

YouTube had changed it's api, which is currently / recently causing issues for users of the YouTube widget.

I inserted the videos into slides. The videos are either .flv or .fl4 files. Thanks,
