Captivate 8, Multi-SCO package - no menu created

I'm using Captivate 8, publishing a multi-SCO package as SCORM 1.2. When I upload the zip package to the LMS, a menu is not created. Therefore, you can only see/launch the first file in my .zip package.  Any idea why there's no menu created?  All files were published as SCORM 1.2 and the final combined zip file also published as SCORM 1.2.
