Captivate 9 launch issue in SuccessFactors

We are experiencing some issues with Heather Spontak's Captivate 9 files Launching.  In the Captivate SCORM package, there are two HTML files, Index_Scorm and multiscreen. The launch file should be multiscreen.html; however, we keep receiving the error message attached when some files are launched and it is not always consistent.

When index_scorm is launched sometimes it will open, sometimes it will not, it is inconsistent.

We are unable to determine if this is a Captivate or LMS Issue.  Has anyone else had this issue?  Any suggestions?
