Captivate Error

When using our link we use pointed to the index.html within our MySQL database the test runs just fine, but when the user enters name and email it gives a captivate error after submission. Interestingly, it works when using link from local machine and it creates the XML file within our database.

What needs to happen: Be able to provide the link from our MySQL database and the user takes the test and the XML file is uploaded to the database. Now our database is Unix. Will that be a problem ?
