Captive 9 Quiz Scoring Issue Incorrect Percentages

Hi,  I recently converted a Captivate 7 course to Captivate 9. I am now having scoring issues on a simple 10 question quiz that includes hot spot, multiple choice and T/F question types. I have triple checked my settings for each quiz question and they seem correct to me. 

When I test the course in Preview Mode or my LMS (Meridian),  I get the following:

1 wrong = 85%

2 wrong = 80%

2 wrong = 19%

3 wrong = 73%

I could keep going, but you probably get the idea.

When we test the course, we do not go back to my practice questions.  I checked those and they are set to Survey. So I don't think the practice questions have an impact on the scoring, although I have heard that has been an issue in the past.

My passing score is set to 80%.
