Changing the text of the "post results" box

Hi I just finished my first quiz in captivate and connected it to my webserver. This worked out pretty well but I am still having a problem with the "Post Results" Box that opens at the end of an quiz and asks you to enter your NAME and EMAIL. I would like to change the text in this box, is this by any chance possible?

I'd like to be able to change the text in the box as well, and also be able to enter my own fields that people have to fill out, rather than just name and email....
