Creating a fillable stamp for company wide use

I am trying to create a stamp which allows fillable fields.  This will be for use across the company.  The stamp should be able to pick up the name of who is using it through their computer,  The fields I am looking for are.  The preparer's name should pick up automatically from the computer sign in
