Employee forms in Captivate?

Hi All,

My company was recently decided to hold off on implementing an onboarding solution to help us with new hire forms, recruitment information, etc. So, in the interim, I'm trying to be creative using Captivate and our LMS.  I was able to create a separate view in the LMS for external users (so they can't see our company information and training) but they can see things specific for new hires.  For example, a welcome video.

We also have a number of pre-boarding forms/paperwork that we send out in the mail.  However, I want to somehow include them in a Captivate course that I can publish to their LMS site. Then, the results can be pulled via a SCORM report.  For example, we have a certification and authorization form that asks question such as name, address, phone number, date of birth, etc. and then requests their signature.  Is there a way I can use text entry boxes in Captivate to capture this information that will generate in a report?

    Text Entry boxes have associated variables, whether they are reported as you want depends on the LMS in the first place.

    My personal choice for such forms would be Acrobat Pro and a database.
