Yes, a part of the issue is that custom buttons sometimes fail to allow an image to be selected. The other part is that custom buttons sometimes cause Captivate to crash when they are copy-and-pasted to another slide. For whatever reason it is working again now, but previously I was getting a fatal error whenever I tried to copy-and-paste a custom button.

Hi, I'm also having this problem, but I found that if you put the images you want for your custom button into the Captivate default folder (something like C:\Program Files\Adobe\Captivate 9\Gallery\Buttons) then you can choose these images directly in the image selector list when you create an image button. For whatever reason I seem to be able to copy and paste buttons created this way. Hopefully it works for you as well.

Are those custom image buttons? Personally I avoid every copy/paste when possible. When I need custom buttons, will create a style for a shape button (you can perfectly insert any image even in the three states of a shape button) and just apply that style to the shape button. Much better work flow.

If you don't get the error when inserting your buttons via the normal methods offered in Captivate (from the Interactions menu) then I would suggest you stop copying and pasting and just do it that way.
