Feature Request and Bug Report Submission Form

Anybody know where this has gone? I had it bookmarked at Feature Request/Bug Report Form, but Captivate is no longer one of the listed products.

If anybody from Adobe is reading this, my request is to have some nice image buttons representing "more..." and "return".

We met Mr Sinha at the Learning Technologies show in London (just a bit of name-dropping there), and I've ended up with:
