How to create a responsive form question?

I am working on a module which looks at understanding several forms. As an activity I am thinking of using a fill in the blanks question type to have the user fill in part of the form based on a scenario. The issue though is when trying to make this fill in form responsive, since especially on mobile the column headers are very difficult to read because the form has shrunk so much to fit into the screen. I am using an image of the form with the fill in blanks inputs positioned on top of the form.

Is there a better way to achieve this kind of question or should i rethink this activity completely?

I think I would rethink the activity for working with a form especially if it is a paper based form layout you are trying to represent on a slide. If so I might consider creating a fillable pdf form which the learner would download. I have used this approach quite often when creating an activity where the learner attempts to fill in medical or financial forms. All of Lilybiri's questions need answering if you are working in a responsive design. My thought is that this activity would not lend itself to a responsive design.

That is a great idea but I don't think the results of this form could be tracked/recorded by Captivate to pass onto the LMS. It sounds like this may have to end up as an animated walkthrough with follow up multiple choice questions.

Which version are you using?  Which workflow for the responsive project: Fluid Boxes or Breakpoint Views? I wonder if it is a good idea to squeeze such a form in a the small browser resolution of a phone at all. Instead of using the default FIB quiz slide, would opt for using Text Entry Boxes or Scrolling Text interactions. You also didn't mention if those entries do have to be validated, have a score to be transferred to a LMS?

I am using the latest Captivate with responsive breakpoint views. The activities will also need to have a score to transfer to the LMS. I am leaning towards using the built in question slides if possible so that this type of "activity" is straightforward to replicate for training in the future.

I wish you all the best but would never do try myself with a FIB quiz slide, which is a real pain in a responsive project, and certainly with Breakpoint Views. Just hope you provide at least double the time needed for a non-respolnsive rescalable project. I don't know if you have created a lot of responsive quizzes with Breakpoint views and had them display in an acceptale way on all screens? If yes, go ahead. If not be prepared for very intensive testing and frustrations.

Since you need transferring results to a LMS, FIB quiz slides have no partial scoring. Score is only obtained when all fill-ins are correct.
