How to edit Captivate 2019 Print Storyboard template

The default template for printing Storyboards in Captivate is pretty close to what I need.
The change I need is on the individual slide listings pages to display the Slide Notes between the Properties and Objects listing.

Example Storyboard template Slide Listing :

Slide1
Properties:
Slide Notes:
Objects:

What do I put in the Storyboard.doc template to have the Slide Notes list where I want them?
