How to get SendTrackingDataAtEnd template?

We're using Captivate 5. We're having some problems getting our LMS's "question-level reporting" report to work. (We want to the report to show the answers that the learners gave to each of the survey questions.) I read this blog report --

-- that explained that quiz reporting may improve on your LMS if you used the SendTrackingDataAtEnd template.

However, the instructions send you to this Acrobat page to download the template:

...but I see no template there.

A couple of questions:

• Do I really need this template to successfully report from a SCORM 2004 course that uses survey questions?
• If I do need this template, where can I find it on the Adobe site? Wouldn't it be on a Captivate page rather than on an Acrobat page?

Has anyone run a report with answers to survey questions?

If anyone can provide insights or experience regarding reporting on survey questions or

regarding using the SendTrackingDataAtEnd template, I surely would appreciate it.

P.S. Could it be that my Captivate 5.0.3.631 already has the improved SendTrackingDataAtEnd template, because I see the date of the file on my C drive is 5/17/2011? See attached picture.

If so, perhaps I need only to make the selection from the Template field on the Quiz Reporting window?
