Inaccurate SCORM reporting in Workday LMS

I have created a number of quizzes in Captivate 2019 and published each as a SCORM 2004 V4 file for use in Workday Learning LMS. One particular package containg a 5 question quiz reported 2 quiz questions inaccuralty. The inaccuracy is strange in that the questions and answers are completely different from what are presented to the learner. The questions and answers reported apear to belong to either another quiz or were the questions in the Captivate file prior to being replaced during the creation process. 

 

Has anyone experienced a reporting anomaly such as this before?

 

I did test the SCORM  file in a sandbox enviroment in a new course without any reporting issue. I also changed 2 questions, published and replaced the SCORM file in the LMS in an attempt to see if I could recreate the problem. However, once the modified file was uploaded and attempts were made the new questions and answers showed up in the reporting. 

 

Any help or insight is apprecated.

    Go to the Project > Advanced Interaction dialog and look at the Objective ID and InteractionID for each quiz question.  Look for any inconsistencies.  Ideally all questions should belong to the same Objective ID, but they should have unique Interaction IDs.  

    If this project file is just a Save As from a previous project CPTX, then you may have quiz questions here that have the same IDs as a previous project that also resides on your LMS and that might explain why you are seeing answers that do not belong.  The LMS is pulling up data from a completely different quiz and interactions.
