Incorrect feedback response is not displaying

Using Captivate 5.5, we have an issue on some questions where the Incorrect feedback caption is not displaying when users submit specific incorrect answers. The question are pretty basic. Four option multiple choice marked to shuffle answers. Submit, Back, and Clear buttons. Captions marked for Correct/Incorrect and Incomplete. User gets one attempt to answer and OnSuccess and Last attempt are marked to Continue with presentation.

But when users select certain options the Incorrect caption does not show and the course immediately skips to the next page (per the Continue option). What is strange is that the option that triggers this behavior is which ever option is in the first (A.) position in the Authoring environment. So if you have the following options listed in the Authoring environment with C being the right answer.

A. Red

B. Blue

C. Green

D. Yellow

After publishing, if a user selects Red regardless of what position it is displayed to the user due to shuffling options - the Incorrect caption will not display.

Users selecting other incorrect options or the correct one get their feedback caption and then continue as expected.

What is causing this behavior?

Have you by any chance removed or replaced the Question Title or Question Text caption from any of the quiz slides? To check, try hiding all objects on the quiz slides.  If the question title or question text captions disappear on any slide, then you will need to replace them with new slides.

The only objects on the page are two images. One is the page background. Hiding them had no effect on the question or option text.

The question title has been removed as it was not needed, but that is the case for all our questions. Many of those without titles do not exhibit this behavior. The Question Text has not been removed or replaced. What does the existence of the title have to do with the incorrect response caption not displaying?

Captivate NEEDS those Question Title captions to be present. You're just going to have to trust me on this. Take them out, or replace them with identical standard captions and your quiz will start to exhibit glitchy behavior. They may LOOK like they are just ordinary text captions but the question title and question text captions are actually special objects in their own right.
