Incorrect Quiz Scoring

Recently we have noticed quiz scoring is randomly incorrect. For example, when a quiz max score is 200 points, some users will show out of 1000 instead of 200 causing them to "fail". See my example below. I find no correlation to the number of attempts or versions, etc. Is anyone experiencing this? Is there a known issue?
