Interactive video in timeline.

There is a lot of discussions on using interactive video to place objects, click boxes etc. over the video to pause, allow interactions, and continue.  But how can you place selected objects, hot spots etc. over a video when you cannot see the video using the play bar (only from the play button).  For example, you can't place a click box over something in a video, if the video jumps back to first frame when you try to add the hot spot.  How do you know where to position it?  Has anyone found a way to author around this.  Thanks.
