Login page issue and reporting query

Hi,

I am currently creating a final test using question slides in Adobe Captivate 4. I have the following queries:

1, Before the user begins the quiz, I have created a login page where the user needs to enter his name, company id and manager id (to be displayed on the reports page). I use advanced actions to do the validation on slide exit when he clicks on submit button, which takes him to the first question slide. This was earlier working on my machine when I publish/preview the file. However, now nothing happens when I click on submit button. The same published file works while testing on another machine. Then when I tested on my machine, it worked after several attempts and now has again stopped working without any change to the published file. What is the reason for this?

2. Is there a way where I could show the questions the user has answered incorrectly in the final reports page? Currently I am not using an LMS, so require the user to send the report by email.
