My custom Quiz template is not showing up, instead it's a hybrid between the Blank Template and my custom quiz template

OS - Mac, OS X El Capitan 10.11.3

I used the Blank template to create a custom template for the MCQ,T/F,FIB,Sequence Slide. When I try to use this as a template in my project, it appears as a weird hybrid between the original Blank template and my template.

Using the Responsive Project, Blank Template to build upon my custom template.

How do I correct this issue?
