Need to embed data entry/echosign style form into capitvate 9

Hi all,

As eSign (formerly Echosign) is an Adobe product, is there any way to embed an echosign/eSign form into an eLearning course, so that the course taker could enter some data and have it send directly to the course manager/administrator/designated email? So say an Induction course for new starters at a company who could then securely enter their contact details and submit them?

I know it's possible to embed eSign into a web page but unsure about how to do so (or even if I can) for an eLearning course.
