Question Slide: drop down showing broken image on HTML5 output

I get the broken image showing once the page has loaded after it has been published and uploaded in our LMS. 

That is a fill in the blanks question slide wherein I opted to   have the drop down.  

Any a dvice or help regarding this would  be very helpful. Also I am a MAC user and viewing this using Chrome. I currently have a Captivate 9 installed on my machine.

Have you checked whether or not your LMS web server is set up correctly to allow JSON files?  They supply the PNG images that are used for a lot of these types of interactions. Ask your server administrator.

Hi Rod,

I've uploaded other modules using the fill-in-the-blank question to the same server without any problems so JSON is working correctly on the server. I've uploaded to SCORM Cloud go the same result. I've rebuilt my development machine from scratch which fixed some issues but not this one. I'm coming to the conclusion that I will need to scrap the fill-in-the-blank question and replace it will a drag and drop in the blank question which I've used before.
