Questionnaire via mobile phone

Good day...I am new to Captivate and would like some help.  I compiled a questionnaire in Captivate and need to distribute it to mobile numbers.  I then need to get the response back on clicking on the submit button.  My question:

1) How do I send it to mobile numbers....do I need to publish it as an app?

2) How do I tell the submit button to send the results back to me.
