Quiz not reporting completion

Hello.  I have created a simulation in which the click boxes have been included for quiz reporting.  When I obtain a passing score, it does not register completion in the LMS.  I launched the course in ScormCloud and it showed "completion."  Can you look at my settings and tell me what I'm missing?
