Quiz slide won't work when published for LMS

If I publish my module as a swf only, with no recording of the quiz scores or  interactions, the quiz slides perform just fine.  If I publish the file as swf/html with the recording of quiz interactions setting on, the quiz slides do not work.  You cannot select an answer; it's frozen.

Is there anyway to remedy this situation?

    Have you by any chance removed or changed some of the default captions and buttons on those quiz slides?

    E.g. Have you removed the Question Title and Question Answer captions?

    You can uncheck the option to show the Clear button, never delete an object that is embedded. The Clear button is not indispensable, since you have to choice to show it or not. Deleting is not a good idea.
