I am using Captivate 9- I have created a quiz with infinite attempts.  when I fail the test the first time on the "score" page I see the "retake" , "review" and "continue" button.  I click the "review" button go through quiz and when I get to the "score" page the "retake" button in not available so I cannot retake the quiz.  What am I missing in the set up?

The design of Retake/Review is this: the Retake button remains active as long as you do not use the Review button. Review button should only be used when all Attempts on Quiz level are exhausted. To prevent that the user is confused, I recommend to drag the Review button under the Retake button (can be done on the master slide for the score slide).

The Continue button will launch the actions you specify in the Quiz Preferences On Success and On Failure. But the last action will only be executed after the last attempt on Quiz level. Since you have Infinite attempts, the action On Failure will never be executed, the user will never be able to get on if he doesn't pass the quiz. Is this really what you want?

Is this something new with Captivate 9?  I have always offered the 3 buttons and when I test the class I fail and then pass- but that was when I had only 2 attempts- maybe that is the difference?

Not new at all, that has always been the case (those articles were written many years ago). Indeed, as I explained, when the attempts on Quiz level are limited, you can have the Continue button working when Quiz has failed after the last attempt. What I explained about Review and Retake has always been the same.
