SCORM file sending multiple completions to our LMS

We are having trouble with SCORM files sending multiple completions to our LMS.  We are using Captivate 2019.  I've attached the settings for publishing the file as well as what the LMS is seeing as multiple completions being sent over to them.  Any insight is greatly appreciated!

Please give the full version number. CP2019 had 3 main releases (11.9, 11.5 and 11.8) and more patch releases. You find the full number under Help, About Captivate.

Did you test the output on SCORM Cloud and get the same result? It is necessary to know if the problem is with the LMS or with Captivate. You only provide one screenshot of the setup. I never did see it based on the number of slides viewed. Some LMSs don't evaluate such a criterion in the correct way. That is the reason for the test I propose.

Forgive my ignorance, I had to learn about SCORM Cloud.  I did create a trial account and publish the course there.  I took the course and completed it but I'm not sure where to find in the different properties if multiple completions were generated after it was complete.  I have attached all of the course settings and the first page is the full version number of Captivate 11.5.1.499  - thanks for any additional guidance you can provide!

First recommendation: update ASAP to version 11.5.5.553. Your version was the first release of 11.5 but had some bugs. Whereas version 11.5.5.553 is the most stable version I ever have used in two decades.

If SCORM Cloud didn't report any problem, it looks like a LMS issue.

I already mentioned the criterion about slides. Some LMSs require a slide to be visited to the last frame to count it as being done. If you have any interactive object like a Next button, some slides will not completely be visited.

What will be functional in all LMSs is to base the criterion on a score. Even though you don't have any quiz, you can add a button which needs to be clicked by all learners and attach a score to that button. The criterion can than be 100% pass score.
