Simulation-failure captions won't display

The failure captions won't disply in a simulation, only the hint caption. I removed other interacitve buttons and it still doesn't work.

    I'm having the same issue.  I have 3 smartshapes on my first slide that are set to display for the rest of the project (pause and CC buttons).  If I remove the "Place Object of Top" checkmark, it resolves the problem with the failure captions, but then I lose my smartshapes.  Note:  the smartshapes are not aligned over of any of the failure captions. 

    Any tips would be appreaciated.

    To add a little more info:  it's not just the failure captions that don't work.. it's the "incorrect choice" functionality of the sim assessment altogether.  I can click anywhere outside of proper clickbox and it ignores the selection (i.e., no failure caption, no moving on after 2 failed attempts).  But I can still click on the correct areas to advance without penalty--still score a 100%.

    Still having this issue on 2 separate projects now.. any suggestions would be appreciated, as we now are going to have to delete functionality to just get the failure captions/scoring working.

    Ensured attempts was set to 2, yet 10 random clicks still produces no failure caption or continue action.

    What are your settings for each of the click boxes in the simulation?  If they are all set to Infinite attempts then there will be no Failure captions because you will never get to the last attempt.

    If you want to see immediate failure captions, set the number of attempts to 1.

    This may be obvious first question, but it isn't being covered by another object, image, etc. is it?
