Survey options reporting as undefined

I have a Captivate 7.0.1.237 survey with multiple response questions.  One of my slides is reporting all of the options on the page except for two of them.  Those two get reported as undefined.  I deleted the slide and recreated it and I'm still having the same issue.

This slide has 11 options.  It has the most options of all of the survey slides, but I though the limit was 15. What can I do to ensure that all options are reporting correctly?
