Trying to use Captivate to collect responses such as SSN, date of birth, address, etc.

My company is not approved to purchase an e-form system/software so I'm trying to use Captivate to capture information that we need from new hires for pre-boarding purposes.

We need information such as their name, address, date of birth, social security number, etc. so there will never be a "correct" answer because this is obviously different for everyone.  What I have done so far is created short answer quiz question slides that are set to the "survey" option (not graded) so that all responses are acceptable.  I published this in our LMS and was successful in pulling the SCORM data from reports (i.e. it lists the question and the user's response, such as their name).  However, I need to know if there is a way to validate that a user entered the correct amount of characters for a particular question.  For example, in the screen shot below, I need to validate that a user has entered their SSN in the correct format (as pictured in the parenthesis) with the number of characters specified.  When we were testing, a coworker said he was able to enter his SSN two numbers short.  I want to find a way to validate that information so we are not obtaining incorrect info.

I hope this makes sense. If there is a better way of going about this, I'd love to hear your input!  I wasn't sure if TEB would work because I don't know how to get the user's entries to populate in a report.

    Please don't use Captivate for this. You're collecting personally identifying information (PII) through a platform that is not set up to secure such information. It puts employees at risk for identity theft, among other things.

    I realize that you have to collect this information somehow, but is there a way you can remove that portion of the onboarding process from your Captivate course? Talk to whoever in your organization is responsible for web development, tech support, or information technology, and ask them to help you identify a secure way to collect this information from employees.
