Video won't launch until player has been refreshed in Captivate 5.5

I'm encountering a really strang issue.  I have a training that is mostly slide video, divided over about 6 slides (video is roughly 5 minutes long, resolution 480x360).   When I launch the project, the project will play but the video won't.  Then, if I refresh it, the video loads and plays just fine.  The video is in flv format, slide video, and I've tested it with the preloader set to 10, 15, and 30%.   Any idea what might be causing this issue?  I've tested it out on two different LMS environments and multiple computers and the issue is consistent.    It plays great once I refresh it, it's just getting it to load the first time.  Any help I could get would be greatly appreciated.

I guess the only advantage of using slide video is if you want it to appear in the TOC. Is that the case?

If not have you tried inserting it as an event video? You may even consider the .swf format for a 5 min video, although this shouldn't be necessary.
