Wrong Answer on a Quiz Question

I have one particular quiz question and I have verified multiple times that
the correct answer is checked in Captivate. Yet, when I answer this
question with one of the other choices, it's marked correct.

Could this be because I marked the option to shuffle the answers on the
page? All the others appear to be working as they should, but this one
question is wrong and the quiz is letting the user pass.

How would I fix this? Using Captivate 3.

Sorry .. it's a multiple choice question and yes .. they are graded. But
this one particular question even though the correct answer is marked, I can
submit one of the other choices and it does not fail. Not ANYone of the
other choices .. just this particular one.

I have the answers set to shuffle so it does not always appear in the same
position, but it is always the same choice. It's as if this one question
has two correct answers .. which of course is not possible.
