Request e-signature not working

I cannot utilize request e-signature all of the sudden.   If I use the app I get a file upload error (screenshot below).  If I try in a web browser I get a "unable to process request" error.  

I was hoping it was just a glitch with adobe when I tried yesterday but it's still not working for me.  I really need to send a document out.  Any help is appreciated.
