Another HTML Help Queestion

I'm getting a little closer on having the Contents listing of my Microsoft HTML Help file looking the way I want it.

Here's what I want (from one done the "old" way)

Selecting the "Printer Properties" "book", displays info on Printer Properties.

Opening the "Printer Properties" "Book" but selecting Printer Properties displays the same screen as above.

But clicking on one of the sub topics displays info on it (as expected)

But when I try to get the same effect with one converting from FrameMaker, I get this:

In the Contents tab, the "Book" is repeated for each entry, rather than the topics subbed under one book entry. Also, what's that row of stuff above the main window?

Ideas appreciated!
