FM 12 to Microsoft HTML Help Resources?

Are there any resources (Tutorials, guides, whatever) for getting a desired appearance in a Microsoft HTML Help (chm) file? I've got an FM12 document that looks the way I want it (except the Table of Contents - this isn't cooperating!), but obviously I'm doing something wrong in the conversion settings since the chm file Table of Contents doesn't look right either. (I want it hierarchical.)

I understand that the conversions uses the TofC of the FrameMaker document as a starting point. One of the problems is that I don't want the FMP TofC to include page numbers (the FMP document will only be available as a pdf for electronic viewing.

Is want I'l looking for clear to anyone? Any ideas?

Did you set your TOC preferences in the Publish > Outputs > TOC/Index Settings > TOC Settings tab?

This should generate a TOC with no page numbers.
