Header and footer not displaying in Body pages

UPDATE: I figured out the issue. I had to apply the Master Page Usage to make the Header/footer show on the 1st page, and modify the height of the header area on the 2nd page.

I have an issue where the header and footer of one page in my document is not displaying in the body page. In the master page, the header and footer appears to be correct, compared to other documents within the book that are not affected.

This issue is only affecting the first page in one chapter, where no header or footer appears. The second page is missing 1 line of text from the header (the header contains 3 lines of text), but the footer is ok. The remaining pages in the chapter are ok, and all other chapters in the book are ok.

I have tried importing formats, element definitions, and running a custom script that I have for my documents, and can't make the header/footer work on the page.

I am using FM 2017.

Any advice?
