How can I fix my template so that body pages are automatically generated as needed?

I have created a FrameMaker template.  It works in ever way except for the fact that it does not automatically generate new body pages as they are needed.  What do I need to change to make this happen?

    Are the Flow A text frames on all Master Pages set to auto-connect?

    Yes, they are. I just re-checked them.

    Is the last Body page by any chance a disconnected page?
