HTML5 OLH Index displays some entries as file names rather than index marker text

I am using FM2022 to produce HTML5 CSH via Publisher. The FM and PDF versions of the generated index look fine, but some secondary entries in the HTML5 index are displayed as book file names rather than the index marker text they contain.

I have attached a screenshot to illustrate the point. I haven't seen this before even though I've published the same book many times before in the past, using the same index markers and same version of FM.

Isn't there an option to add the file name? I do not know (without FrameMaker here), if this is a general option, or whether you can set this for individual markers.

You can configure settings in Publisher to use the file name, but I'm using <paratext> for topic titles. Thanks for the suggestion.

Upon further examination, it appears that this behavior happens when a single index entry references more than one page number (separated , in my case, by commas). See the attached file.

As a work-around, I re-worded the index marker text to make the entries for each page unique.
