I have a book with chapters that are numbered. I would like to have the chapter number and page number (separated by a hyphen) appear as the page number in both the TOC and the Index. I have already generated the TOC and Index and the pages are just numbered from 1 on in each chapter.

You have to add the <$chapnum> building block to the TOC and IX definitions on the Reference pages. See: http://help.adobe.com/en_US/FrameMaker/12.0/Help/Using/using-framemaker-12/frm_books_bk/frm_books_bk-9.htm

You also then have to set via the book file the chapter numbering behaviours for the component files.
