Lose booksmarks when pdf FM11

I need urgent assistance! I am working on a FM11 doc that was originally created in FM8. When I try to print to pdf there are no elements in the element list and I get no bookmarks generated in the pdf. We are racking our brains!

If you've got these in a book, FM usually uses the first file in the book to obtain all of the required info. Check the first file.

When you run the PDF Setup and click the Bookmark Source to Elements, are you saying that no elements are available in the selection lists?

Perhaps the document isn't structured?

If you are using the Print function, you must have the Generate Acrobat Data function enabled in the Print dialogue to get bookmarks.

It is recommended to use the SaveAsPDF function in FM11.

Correct - no elements show up. Here is a screen shot:

It is definitely structured. As you see in the screen shot the Generate PDF Bookmarks is checked.

I've tried both Print Book and SaveAsPDF and neither work. I can save individual .fm files as PDF and they have the element sets, but when I try to combine all the individual PDFs I lose the cross-references, but the bookmarks are there. We have to deliver the book with both bookmarks and cross-references.
