PDF-creation from book - mixed page sizes

Currently i find no way to be able to have A4 files and A3 files in the same book and get an output according to that. All A3 are cropped to A4.

EDIT: I could make this wor by making a PS and Destill it.

Those damn paragraph to bookmark settings keeps changeing - whi cant they just save?
