Spanning the alpha characters In the Index across both columns

I have a two-column book Index file, and I have forgotten how to span the Alpha (A, B, C, D, etc.) characters and title across both columns.  Can someone assist me with that?  Matt Sullivan showed me how to do it, but I cannot find it.

    Sorry!  I managed to fix my spanning issue, but maybe not the best or recommended method.  I went to the Paragraph Designer, selected the tag for GroupTitlesIOM and changed the tag to Across All Columns and updated.  Visually it appears to work just fine.  Probably a more effective method is to update the Reference pages for the Index, but it is really not set up, and I don't have time to update it for now.
