TOC has extraneous words!

I generated a TOC in 2015 FM. I am getting extraneous words in it like NOTE:, TIP:, etc (attaching file)

    Check the TOC Reference page and remove the <$paranum> entries from the various paratags that you have specified for the TOC.

    All of the weird ones are used as autonumber text in another tag.
