unwanted bookmark when generating .pdf

* Distiller 9.0 (as part of Pro Extended); FM 9.0p237

It's a book I haven't had to look at for the last two years, and then suddenly They want an update for the end of the day ... could be worse, as I had at least seen it coming. All updated, then, but there's something strange going on with the .pdf bookmarks: they're picking up :h0TOC entries from the generated Table of Contents, while there's no sign of :h0TOC in either the Include or the Exclude panes of the PDF Setup - Bookmarks tab. This is odd, as I can see the :h0TOC entries from the generated Table of Contents where they should be.

Any tips on how to suppress the unwanted bookmark in the output .pdf?
