Click File > New > Document, the New window is opened. At the lower left, there are buttons called Explore Standard and Explore Structured. If you click Explore Standard, you'll see a set of templates for most of what you're looking for. Includes a cover page but does not include a page for LOT/LOF. TOCs/LOTs/LOFs are pretty much the same, though, so you can use the TOC to create the others.
The Frameusers site has the old Adobe templates (from version 5.something or other) available here: https://www.frameusers.com/tools/
These templates do not come with a front or back cover and like the above only have a TOC page in addition to the regular page. They also don't include Index pages.
I thought the template packs were still available from Adobe, but I can't figure out how to get to them.
