Color swatches opening up blank after creating in Illustrator

I have several custom color swatch palettes that our artists use in their artwork created in Illustrator. Every so often these files need to be updated with new colors or changes to existing ones. Sometime last year I lost the ability to edit or create these swatch files. If I create a new swatch file from scratch, it will open up blank after it's saved, with no colors listed. Other times I've had it crash my computer just trying to save the swatch file. I have also had it open up but only show the CMYK values instead of the color names after I try to jus edit an existing palette, which drives the artists crazy. It appears random, but others in the art department are having the same issue I am. Any ideas what might be happening? Below are some screenshots of what I am seeing.
