Flatten transparency getting more than 27 spot color error

I am trying to flatten this image (or any other 1clr image or 1clr imported pdf) and I keep getting the 27 spot colors error.  The image is only 1 color and is the only thing in the file.  Is there something I am missing?  It used to work, but now it doesn't.
