Gradients corrupting

How do I solve this issue:

If I Have a gradient in a vector and I

1. paste it into photoshop or

2. if I send the document and someone else opens it

The gradient changes to black/white and sometimes keeps one or two color (but not the right ones).

And plot twist, it doesn't happen with all gradients or all the time (probably 75% of the time)- and I can't find a pattern to when the gradient works and when it doesn't.

I have NO idea how to fix this issue and I've tried just about everything.  Anyone ever have this issue/knows how to fix it?

Illustrator -- it's a simple vector graphic and filled with a gradient -- it's not flattened

Which software are you using to create the image?

Are you flattening the image layers before you send it?
