Help getting a photo in a polygon shape for a banner image

I am having a huge dilemma!!  I created a design to use as a banner for a display next month.  The company that is creating the banner is requiring us to save the phoo with these dimensions in Illustrator.  I have no idea how to do this.  Can someone please help!!
