I have a white box showing on image but doesn't show on a PDF proof only when open in AI , client that sent file doesn't see it

Hopefully the device or RIP application doing the final print work has an Adobe certified PDF Print Engine. Otherwise those boxes that are visible in Outline View may show up as faint discolored boxes in the actual print output. Some call it yucky discolored box syndrome.

Doug thanks for your reply , I'm using latest version, funny thing is I opened and use this file in the past and it wasn't there,  I'll dive in and investigate hate when clients send a mess thanks

I don’t see it either. Which version of Illustrator are you using?

There are a lot of nested clipping masks in the file though, including some that correspond to your white box. Try removing some:
