Illustrator creating solid filled box around and ontop of text when outlined

I've witnessed this twice now. I can't find any rhyme or reason for it. When I outline files to send to print, I unlock all layers > select all > create outlines.

I've now had 2 instances where a box the color of the text appears on top of the outlined text, with the text outlined underneath it. I can't find anything about this anywhere. Is this a glitch?

Thoughts?

Are they "Point" or "Area" type objects? Is it possible an overprint "fill" is assigned to the "Area" type rectangle and is losing the overprint when outlined?

I don't see it reported as a bug on Uservoice. You can report it, but you might want to make sure something else isn't going on. It's hard to tell what happened without more information like if there were any other steps you took or what all was on the various layers.
